Your First Visit

We want to make your first visit to Market Street Animal Clinic comfortable and stress-free. Here’s how to plan your visit.

When you check in at our reception desk, we will ask you to fill out a Pet History form so that we have relevant information on your pet.

When you visit us for the first time, be sure to bring your pet’s medical and dental records, including records of shots and surgeries, from your previous veterinarian. If you adopted your pet from a breeder, animal shelter or animal rescue, please bring all the papers that were provided to you at the time of adoption. If your pet had a previous owner, bring the records that were supplied to you by the other owner.

We will need to know if your pet has any allergies to food, environment or medication. Please also tell the vet any other information that you feel is important regarding your pet and their health, including recent changes or possible stresses at home.

Prior to your appointment, we recommend that you jot down any concerns or questions that you have so that they are addressed at your appointment.

Pet Portal – Our Pet Portal puts pet health information at your fingertips. You can connect with your veterinarian 24/7, view your pet’s health records and vaccination history, even upload photos of your pet. Search our extensive pet health library and much more!

Online Pharmacy - Purchase your pet prescriptions and preventatives on our convenient online pharmacy and have everything delivered to your door.
